One of the most critical parts of a workplace safety program lies in performing a thorough investigation into why an accident or incident occurred to prevent future accidents that could result in the same or similar outcomes. 

Workplace accident investigation processes and procedures to determine root causes should be methodical and comprehensive, and traditional root cause analysis and OSHA’s incident investigation processes meet these objectives. When properly structured, implemented, and followed, both approaches—especially when combined—have proven to be effective and efficient method for performing investigations and reducing future accidents.

He has a B.S. in Chemistry and a Masters of Public Health. He was a NIOSH traineeship recipient (1995-1997) and is a member of ASSE and NAEM. His certifications include OSHA 30-hour training (2012), and ISO/OHSAS Internal Auditor Training (2012).
